​Dr. Fayad earned his DDS from Cairo University in 1985, followed by a Masters in Oral Sciences (1994) and PhD (1996) from the University of Buffalo. He completed Advanced Education in General Dentistry at Eastman Dental Center, Rochester, and a Specialty in Endodontics at the University of Illinois. Currently, he directs Endodontic research and is a clinical associate Professor at UIC's College of Dentistry. He divides his time among teaching, research, and private practice, serving on advisory panels for top journals. Dr. Fayad co-edited "3-D Imaging in Endodontics: A New Era in Diagnosis and Treatment" (Springer, 2016) and holds Diplomate status in the American Board of Endodontics, presenting widely.

While Navident is traditionally associated with implant treatments, this case report demonstrates its valuable application in both complex apicoectomy and root canal procedures, emphasizing precision, efficiency, and versatility in improving surgical outcomes.

Case 1: Apicoectomy for Tooth #31

A patient presented with a low-density area around the mesial and distal roots of tooth #31. Due to completely calcified canals and limited mouth opening from a previous jaw surgery, an apicoectomy was planned.​

Navident's treatment planning software was used to plan the buccal lid technique, visualizing piezosurgical saw cuts (yellow lines) to avoid existing surgical pins, showcasing the system's precision in surgical planning.​

Coronal and sagittal views of realistic renderings, utilizing a bone filter, showed the proximity of dental screws to the distal root.

Surgical Procedure

The buccal flap was reflected, revealing the dental screw. Guided dynamic piezosurgery was performed , and root-end resection was achieved without damaging the screws, highlighting the system's efficiency in achieving surgical goals.

Post-Surgical Outcome

The bony lid was replaced, and a bone graft with a collagen membrane was applied. Immediate post-surgical CBCT scans showed successful bone graft placement and intact dental screws, confirming effective bone remodeling and surgical accuracy.

Case 2: Apicoectomy for Tooth #14

A patient presented with a thick bony exostosis (15 mm) on the buccal palate, necessitating an apicoectomy for tooth #14.​

Navident’s real-time dynamic navigation was used to perform the osteotomy towards the disto-buccal root, ensuring precision and minimizing tissue removal.

Surgical Procedure and Outcomes

Root-end resection and filling of the mesio-buccal and disto-buccal roots were completed (Figure 10a). Post-surgical CBCT views confirmed apical and buccal cortical plate remodeling, indicating successful treatment.

Case 3: Root Canal Therapy for Tooth #14

An 80-year-old patient required root canal therapy for tooth #14, which presented with pulp and root canal calcification (Figure 11a). Axial and coronal views indicated a traceable canal in the apical third

Real-Time Navigation and Treatment

Navident software imported the CBCT DICOM images for real-time treatment planning and execution. ​

The dynamic navigation system enabled conservative access preparation, preserving tooth structure while locating the canals with minimal removal, demonstrating the system's efficiency and precision.
